Midland School is Nut Aware
Midland School has many children with life threatening nut allergies. We ask that you refrain from sending any Peanut or Tree nut products to school for snack or lunch . Nut products will NOT be allowed in the classrooms. If your child does bring these items to lunch they will have to sit at the "nut table" in the cafeteria. Please speak to your children about the severity of these allergies and how they can affect some of their friends. We understand that these foods can be healthy for some children but , they can threaten the lives of others. Thank you for your cooperation with this issue and helping us to make Midland a safe place for all.
New Student Parents
Please see new student forms. All forms must be in the health office before your child can start school. Health appraisal forms must be done in New York or the tri- state area. All required immunizations must be done before your child can enter school.
2nd and 4th Grade Parents
The health appraisal forms are required for entering 2nd and 4th grades. Please see these under general health forms. Health appraisal forms for the 2015-2016 school year must be dated after 9/ 8/14 . New students must have a copy of their immunizations.
Medication Forms for 2015-2016
Parents of children that require medications to be given in school or be in the health office for emergencies: You may print out the forms under Parent and MD authorization forms. Please return the forms and the medication to the health office by September 8th 2015. If your child requires an Epi-Pen for allergies please come by the office to pick up the forms that your doctor will need to fill out.
